Are pierced nipples hot? Or not worth it?

Hot or not... tastes are different. The only relevant question is do you want to have yours pierced?
A former work colleague who never failed to flirt with me had both of her nipples pierced which was pretty obvious to see through her shirt. She told me, wearing her piercings created a constant stimulus that kept her constantly horny.
So if you are planning on some naughty/kinky Hotwife playtime out in public, having your nipples pierced will probably improve your experience. But probably the best thing about it is: you can always decide when to wear your piercings and when not to. You don't have to wear them all the time! But also she told me that before she made the decision to actually have 'em done she had gathered info on the benefits but also what could potentially go wrong. She said that in rare cases the procedure may cause damage to the nerve web in the nipple area which may result in a desensitized nipple which is a big downside ImO when that happens, especially that it is not reversible.
So if you decide for it, in order to minimize risks, i suggest for you to consult a piercer who is experienced with nipple piercings, who values hygiene on his work place no less than a medical doctor would, and who has a good reputation for his work.
A note on aestethics: Again i say, tastes are different. However i noticed that, depending on the natural form of a Woman's breasts, sometimes it looks more appealing for the direction of the puncture (or is it "grooving"? - Srry English is not my first language
) to be arranged horizontally while in some cases it looks more appealing if it's done with a bit of an angle. However ImO the two sides should be done symmetrically! Try step in front of a mirror, hold your most desired nipple charm to your breasts and try to imagine what it'd look like when applied in different angles. Then decide for what you feel suits you best.Whatever your decision will be... keep having fun!

Seeing as piercing the clit or nipples is a desire to add more sensation to the erogenous zones, I believe I could find alternatives to sticking pins through these sensitive areas.
